Online Application for UA Documents

To file your application for documents, kindly observe the following:

  1. Download and accomplish the Request Form for Documents.
    The form may be accessed at https://bit.ly/UA_document_request
    Fill it out via Adobe Acrobat Reader. Tick the box in the form for the purpose to be indicated in your
    document (example for evaluation purpose).
  2. Send the accomplished form to reg@ua.edu.ph for assessment purposes.
  3. Pay the assessed fees
    Upon the receipt of the confirmation email from the Registrar, you may pay the assessed fees via:

The Account No. is your UA Student Number.Transcript of Records (TOR) = Php 200.00
Transfer Credentials (TC) = Php 500.00
Certificate = Php 100.00
Authenticated Diploma = Php 50.00 (Send via email the scanned copy of the diploma.)
2nd copy of diploma = Php 700.00 (Send via email an Affidavit Loss notarized by a lawyer. The
ORIGINAL copy of the Affidavit will be submitted in person upon the release of the diploma).
Course Description = Php 100.00/page
Processing fees (WES, BON, CGFNS, ICES, IQAS, etc.) = Php 200.00
TOTAL:

  1. Send the Proof of payment to reg@ua.edu.ph for purposes of validation.
  2. As for the status of your request, follow up after 3 working days from the day of accomplishing
    step 4

To expedite the filing of your application for documents, you or your authorized representative may
personally visit us in UA every Monday to Friday, 7:30 am -12:00NN, 1:00 pm – 4:30 pm.

Authorized representatives should present the following for the filing and releasing of documents:

  1. Authorization letter with the requestee’s signature
  2. Scanned copy of the requestee’s ID with signature
  3. ID of the authorized representative
